All incoming students are required to electronically submit immunizations and personal history information through a secure web site.
Once you have your PSU user ID and password and have activated your account, you can access the University Health Services website. There you will find detailed requirement and exemption information. *Failure to submit your immunization information will result in a denial of registration for your second semester at Penn State.*
You will be asked to complete certain online requirements including:
- Notice of Privacy Practices
- Your Health History
- Insurance Coverage
- Personal Information
- Screening Page
Prior to opening these pages to complete your entrance requirements, gather your health records, including medications and insurance records. Also review your family members' health history.
